Unusable ladders and too small tunnels

In several places in the game there are ladders that I see but can't use. Eg barren peaks. Also there are tunnels that are too small. Is there any thing I can do to climb these ladders or enter these small tunnels or are they just red herrings?

Just decoration usually.
You have 20 save slots use them all. Save after every quest, save before going into a dungeon, save, save save!!
