Slender Helm

I have everything but the Slender Helm. I've killed every monster, clicked on every coffin. Some don't say anything, others say 'just dust and bones' and some say 'It's locked'. I've killed the ice eye dude twice and got the Unholy Key. I just cannot find the dang helm.
I've even visited every blacksmith in every town in case I accidentally sold it, which I am almost certain I didn't do. Any advice? Can someone tell me the exact coffin it's in? I feel like an idiot.
