I found out in HOL1 that if you kill an enemy standing over a hole (ladders, etc.), the items can't be recovered. Was this intended? Personally, I didnt need the items but it worries me to think that it could have been a quest drop and been a gamebreaker for me.

You need to go through the hole and go back up. Then you will be on the hole for a turn, long enough to pick stuff up. Same with teleporters.

It isn't a bug as such more of a flaw in the game mechanics. I beleive zarista games is/was aware of this so makes a point of not putting quest monsters near stuff like this.
You have 20 save slots use them all. Save after every quest, save before going into a dungeon, save, save save!!
